Independent review services
Each engagement starts with defined clinical questions and an agreed deliverable. The analysis addresses those questions directly, explains relevant supporting and conflicting records, and identifies the limits of each opinion. Care may be supported, partially supported, or unsupported; conclusions are not predetermined. Case fit, qualifications, conflicts, deadlines, fees, and secure record transfer are confirmed before substantive review.
Assess treatment decisions, diagnostic testing, frequency, duration, referrals, and continued care against the documented presentation, function, response to treatment, and relevant clinical guidance. Identify supported care and areas needing stronger justification.
Evaluate specific billed charges using a stated methodology and appropriate reference information. Identify disputed amounts, benchmark assumptions, and data limitations. Charge reasonableness is analyzed separately from clinical necessity.
Consider each Texas Civil Practice and Remedies Code §18.001 request individually for case fit, qualifications, conflicts, and applicable requirements. When accepted, identify the services or charges at issue and explain the clinical or charge-analysis basis and its limitations.
When included in the engagement, assess injury chronology, prior history, clinical findings and alternative explanations. A temporal relationship or abnormal image alone does not establish causation.
Examine whether symptoms, examination findings, imaging, diagnoses, treatment choices, reassessment, and functional progress align. Explain material discrepancies, missing documentation, and the limits of the available evidence.
Offer scoped clinical consultation for spine and injury record questions. The requested deliverable, record materials and questions are agreed before substantive review.
DF Spine Review also considers appropriately scoped Texas spine and musculoskeletal Independent Medical Examination (IME) requests. Accepted examinations are conducted in person at Dr. Farmen’s Irving, Texas clinical office.

About the reviewer
Derek Farmen, D.C., FSBT(c), is a Texas-licensed chiropractor with clinical experience dating to 2018, postgraduate spine MRI education, and a role in clinical instruction at Parker University. FSBT(c) denotes fellowship candidate status.
His approach considers the full clinical record—including history, objective findings, imaging, care decisions and functional progress—without overstating what those findings establish.
